When completing the Clean Water Contact Form, please try to categorize your report into the appropriate Problem Type as summarized, select all that apply. Attachments, such as photographs, can be uploaded

Flooding – to report flooding along the Turnpike roadway
Illicit Discharge – to report any non-stormwater discharges, or stormwater pollution coming from the Turnpike
Sediment and Erosion – to report muddy stormwater coming from the Turnpike
